Department Forms

Here you will find forms for payroll management and setup or removal of services such as keys, building access, and email configuration. All Employee Payroll Setup/Change/Exit Forms need to be given to both Marilyn Burton and Karen Evans in the Main Office.

Employee Forms

New Employee / student Check In Form

Below are links to a Welcome Site and Check In Form for new people to the department. The Check In Form as well as additional forms

  • Welcome Site - A web page for new employees or students informing them of things they must do by the first day in the department and also informs them of U of U services and benefits. This site has a link to the Check In Form (see below) and the government eligibility to work forms. The entire web page is also able to be downloaded as a PDF.
  • Check In Form - For new employees or students in the department to get set up. Please have new employees or students print this form and fill it out by the first day they are in the department and give it to Karen Evans. She will then fill out what she needs to do and pass it on to the others in the main office for completion of the Check In process. When the person leaves the department the process is done in reverse to ensure all email lists are updated, keys are turned in, etc.

Non-Employee Form

Affiliates

  • University Affiliate Form - This is used for people on campus who are considered "Affiliates" or for non-typical employees working/studying/consulting at the U in some capacity for a period of time longer than 2 weeks.. The form on this site is to be filled out and faxed per instruction on the form. Questions regarding affilliate status should be directed to the sponsor of the person in question.

Inventory Form

Off-Campus

  • Off-Campus Use Form - This is used to create a record of University of Utah owned equipment that is used off-campus for inventory purposes. This is to be filled out and printed and then given to Katherine Ebert in the Main Office (401 MREB).
